This information is used to help identify when and where there is a shortage of midwives and will assist the College of Midwives of British Columbia (CMBC) and the Ministry of Health Services in projecting the demand for midwives in the province of British Columbia. Detail is requested so as to identify women who may be on one or more waitlists seeking midwifery care.
With your consent, the information collected is submitted to the CMBC, a designated public body under the Freedom of Information and Protection of Privacy Act, for processing. The CMBC will extract the data required and create a report with all personal information removed. Personal information will be securely stored and will not be disclosed for any reason.
